What it does
RECORD (or remove) a spec-to-code link. Defaults to tier "human": a confirmed link promotes that chunk to "evidenced" — the human half of "a T0 deterministic match OR a ratified human link".
When to use it
Use when: "I confirmed this code implements this spec" · "ratify this link"
Also use when: a human reviewer verified the link
When not to use it
the link is model-proposed/unconfirmed — T1 links come from spec_generate/spec_sync; or the user only wants to READ evidence — use spec_view, which cannot write
