The tool is missing

Check the installed plugin version and the host's tools/list. Rebuilds in the source checkout do not update a versioned installed cache. Reinstall after the manifest version changes.

Results are empty or indeterminate

Confirm the repository root, index freshness, supported language, and analysis caveats. An incomplete index should produce an incomplete result, not a fabricated clean result.

The done gate never runs

Confirm the host accepted the packaged hooks and that STRATA19_GATE_MODE is not off. Codex may require the hook definitions to be trusted; enterprise Claude policy can prohibit unmanaged hooks.

The workspace pane is blank

Confirm STRATA19_WEB_BASE points to a reachable HTTPS origin. Sandboxed host frames may reject local HTTP even when the same page opens directly in a browser.

Read the pending action, approval state, and receipts. A typed unsupported result or waiting_for_human state requires handling its declared boundary; restarting the run should not be used to skip it.

Plugin and web app disagree

Compare project identity, synchronization status, and timestamps. Resolve stale connected state before editing source or specifications to match an outdated view.
